Insurance Coverage
You should contact your insurance company to verify whether Black River Health is in your insurance network. We collect co-pay, coinsurance, and/or deductible payments at the time of your appointment. If your insurance does not cover the cost of your care or respond to us within a certain time frame, your statement may show the full amount due.
Out-of-Network Coverage
If Black River Health is not within your insurance network, you may still be seen by a Black River Health provider. However, you must:

- Use your out-of-network benefits
- Contact a patient advocate to make arrangements prior to your service
- Pay for your services in full when you receive them
- Complete the appropriate paperwork as required by your insurance company
- Ask your insurance company to reimburse you
Insurance Disclaimer
It is your responsibility to verify with your insurance carrier that your plan will cover medical services provided by Black River Health providers.
Some of the insurance companies we contract with do not consider Durable Medical Equipment (such as equipment from Home & Respiratory Equipment) or Care at Home Line of Services as covered services. Please check directly with your carrier to confirm your coverage.
