Care at Home is an overarching term we use to simplify a variety of services offered by Black River Health. The services below can be used either alone or in conjunction with another service to further individualize your care to your specific needs.

The Home and Respiratory Equipment department offers home equipment such as BIPAP, CPAP, and other respiratory items to qualifying patients. Supportive Care

Supportive care services are available to residents of Jackson County. These services are completed by a trained supportive care worker and are limited to bathing, oral cares, incontinence care, hair grooming, ambulation/repositioning, or feeding assistance.

Please note: Supportive care is unlicensed and uncertified with no provider orders or registered nurse oversight. This is a cash-based service menu selected by the resident themselves.

Care Management

Patient and nurse work together to arrange patient care and to communicate healthcare needs and concerns among all healthcare providers involved in the patient’s care, with the goal of increasing safety at home. This service is completed through a combination of in-person clinic visits and telephone conversations.

QUALIFICATIONS

  • Chronic Care Management (CCM) – Two or more chronic conditions lasting 12 months or more.
  • Principal Care Management (PCM) – One or more chronic conditions lasting 3 months or more.

Examples of chronic conditions: heart disease, Chronic Obstructive Pulmonary Disease (COPD), diabetes, high blood pressure, and arthritis.

SERVICES

  • Assigned care manager who will work with you to develop a care plan to meet your healthcare goals.
  • Assistance in coordination between healthcare providers and services to assist with scheduling appointments, lab tests, and other services.
  • A nurse to assist to better understand medications and their effects.
  • Nurses to help answer questions that will arise.

COVERAGE

  • Medicare covered service that applies to deductibles and coinsurance.
  • Covered by most insurances with prior authorization.

  • Help prevent or decrease hospitalization.
  • Ability to contact health professional regardless of day and time.
