Black River Health created the PETAL Award recognition program to celebrate our care team. This award gives patients, families, and co-workers an opportunity to celebrate and thank a care team member for the exceptional care they provide.
Who can nominate a caregiver for the PETAL Award?
Anyone can nominate a member of the care team for the PETAL Award. Nominations come from other providers, clinicians, staff, colleagues, patients, and family members.
How do I nominate a member of my care team?
To nominate a member of your care team for the PETAL Award, simply fill out the form on this page.
Who selects the winner?
Once submitted, your PETAL nominee information will be reviewed by the Black River Health nomination committee, who will select a winner.
Will the PETAL Award winner receive any rewards for winning?
The PETAL Award winner receives a certificate and a celebration in their honor.
