This way, we can update your insurance information and provide you with important information and policies.<\/li>\n<li>Bring a list of your health care questions or concerns. This helps us identify the most important topics to cover during your first visit.<\/li>\n<li>Complete a patient information form. This will help us keep your address, contact information, and emergency contact information up to date.<\/li>\n<li>Complete a medical records release.
